Annual membership includes:

• 2 periodic exams
• 2 cleanings
• 1 emergency exam
• 1 oral cancer screening
• 1 fluoride treatment
• All x-rays (including Panoramic)
• 10% discount on all other treatments (excluding cosmetic treatments, ortho, implants and
  removable appliances)

New Patient Member: $450 the first year of membership (over $800 value)
First family member: $399/year
Second family member/each additional: $349

Terms and Limitations of Membership:

• This plan is non-refundable. No refunds if the patient chooses not to use their membership.
• This plan is non-transferable. Family members cannot be substituted for another family member.
• This is a dental discount plan and is NOT dental insurance. It cannot be combined with any other
  dental insurance.
• Your effective date is the day you sign up, is good for one year, and renews automatically on the
  same date each year.
• Co-payment is due at the time of service.
• Rates are subject to change annually.
• Your membership is good at this office only. Therefore, should we refer you to a specialist, your
  discount will not apply to that office.
• A fee may incur for broken appointments not cancelled 48 hours in advance. Exams, x-rays, cleanings, and
  fluoride must occur in the year of enrollment and cannot be carried over to the next year.
